List of the documents which should be enclosed to the Application

Following documents should be submitted in order to receive the permission for the performance of the construction works:

• Application in accordance with the model form;
• Document which confirms property or enjoyment right, including lease, to the parcel of land;
• Decision of the Kyiv City Council about granting of the parcel of land for the construction purposes or Instruction of Kyiv City Administration on the permission of the construction, reconstruction, renovation and complete repair;
• Complex report of state investment expert examination (respective statement of state expert examination is submitted in case the object is included in the list of the object approval of which does not need complex expert examination);
• Document of endorsement of the project by the Chief Architectural Committee;
• Order on the approval of the project (according to the Resolution of the Cabinet of Ministers of Ukraine of 17.08.98 # 1308);
• Certificate of frontages (original copy);
• General Construction Plan (endorsed by the Chief Architectural Committee and others);
• Order document of the Prime Contractor on the appointment of the Contractor (executor);
• Order document of the Customer on the appointment of the person responsible for the engineering supervision;
• Order document of the project organization on the appointment of the person responsible for the designer supervision (project representative);
• Document confirming ownership right to the building or construction or document confirming the consent of its owner for the performance of the works should be submitted in case of reconstruction, renovation, and complete repair.

Following documents are submitted for the registration:

• First part of the project (explanatory note) with the stamps of Chief Architectural Committee and State Expert Examination (original copy);
• Certificate of frontages (original copy);
• General Construction Plan (endorsed by the Chief Architectural Committee and others);
• Journal of General Works (form СНИП А.3.1-5-96);
• Two copies of Journal of Designer Supervision (form СНИП А.2.2-4-2003).

Order documents on the appointment of persons responsible should contain following information:

• Full name;
• Official capacity (appointment);
• Qualifications (education);
• Address and telephone of the working place;
• Address and telephone of the living place;
• Person responsible should examine the order and sign it.
